Wellbeing, Social Prescribing & Single Point of Access
What is the service?
This service is available to patients of Central Vale Practice patients only and is in addition to the primary care mental health services that Mind in the Vale also provides to patients including the Guided Self Help (Tier 0) and the Fast Track referrals.
General Wellbeing – (someone to talk to, loneliness and isolation)
- Physical Health (e.g. weight management, exercise)
- Social Issues (housing, debt, benefits)
- Education/Employment
- Addictions (drugs, alcohol, smoking, gambling, etc.)
- Pain Management (Fibromyalgia etc.)
It supports patients aged 16+ to focus on achievable goals and access local community resources.
Mind in the Vale will make contact within 5 working days and either provide telephone advice and signposting or offer a face-to-face assessment.
Patients will ideally meet Wellbeing staff in their local GP surgery, meaning they are in a familiar environment and reducing travel. The service aims to ensure patients get the right support in the right place at the right time by the right person/agency based on their needs.
Often patients will have wellbeing as well as mental health needs and as a result of the single point of access service MITV can also ensure referral to an appropriate support (including our Guided Self Help)
